DUI School registration form

12-Hour State Certified Prime For Life Course

Each month, CADAS provides comprehensive educational courses for persons convicted of driving under the influence, driving while intoxicated, underage possession of alcohol, etc. This state certified course educates students about alcohol and other drugs, abuse and addiction, and adverse physiological, psychological, and familial effects.

Each student receives an alcohol and drug evaluation; the results of which are reviewed individually with an A&D professional. If appropriate, we will refer clients to substance use treatment, mental health care, and social services.

  • One 12-hour class is offered twice per month on Saturday
  • One class is offered in Spanish each month.
  • Anyone may take the course: DUI offenders, underage offenders, etc.
  • parents enroll their teenage drivers to learn about the hazards of drinking and driving.

Out-of-State Evaluations:

Today, a person convicted of a D.U.I. or similar crime in another state is required to complete an alcohol and drug assessment. Our staff evaluates clients and work with them to renew their driver’s licenses.

Fees and Scheduling

  • DUI School with Certificate of Completion – $125
  • Out-of-State DUI – $200
  • Rescheduling Fee – $35
  • Late Fee – $35
